Domonic Hopson
Domonic Hopson is the President and Chief Executive Officer of Neighborhood Family Practice (NFP). NFP serves more than 22,000 patients across seven sites in Cleveland and Lakewood, OH, providing primary and preventive care, and integrated services, including behavioral health, pharmacy, midwifery, and dental.
Born and raised in Southern Mississippi, with a single mother who relied on multiple subsidy programs to support her family, Hopson made an early decision to advocate for equity in the delivery of health care to those who need it most. Before joining NFP in 2022, Hopson was Chief Executive Officer (CEO) of the City of Cincinnati Primary Care (CCPC) and the Assistant Health Commissioner for the City of Cincinnati. Hopson oversaw the Cincinnati Health Department’s COVID-19 response from April 2020 – May 2021.
Hopson was recently named a “Notable Blacker Leader,” by Crain’s Cleveland Business, “40 under 40 Leader in Health,” by the National Minority Quality Forum in 2024 and received the Black Professionals Association Charitable Foundation’s 2024 40/40 Emerging Legends Award. He has also been recognized by Crain’s Cleveland Business as a 2022 “Forty Under Forty,” and “New Faces in New Places,” and by Modern Healthcare as a “Top 25 Emerging Leader.”
Domonic serves as a Board Member for ACHE of Greater of Ohio, Ohio Associations of Community Health Centers, The American Heart Association and Collaborative to End Human Trafficking.
Chief Operations Officer
Terrance (Terry) Byrne
Terry plays a key role in facilities management and new operational initiatives. Terry, who began his career as a respiratory therapist, holds master’s degrees in health law from Loyola University and health care administration from Cleveland State University. He earned his bachelor’s degree in applied health from Baldwin Wallace. Before coming to NFP, Terry was the chief operating office at a community health center focused on the homeless and those living in public housing. Prior to that, he served in an administrative capacity at University Hospitals Parma Medical Center and Southwest General Health Center with a focus on health information, compliance and risk management.
Terry is a recent graduate of LeadDiversity and is a Fellow in the American College of Healthcare Executives.
Vice President of Development and President of North Coast Health Foundation
Gina Gavlak
Gina Gavlak earned a bachelor of science in nursing from Capital University. Gina joined NFP in January 2019 as vice president of business development and president of the North Coast Health Foundation. In her role, she leads the development of new programs and services, promotes community partnerships, works to develop business strategy and strategic priorities. Gina brings 27 years of experience in administration and leadership, program and fund development, advocacy and direct patient care. Prior to joining NFP, Gina served as president and CEO of North Coast Health, a faith-based charitable clinic, and led efforts which brought about the strategic affiliation of NCH and Neighborhood Family Practice. As president of the North Coast Health Foundation, Gina works to advance fund development and advocacy efforts to connect and mobilize community support for NFP’s six community health centers.
Chief Medical Officer
Melanie Golembiewski, MD, MPH
W. 117th Community Health Center
Melanie Golembiewski, MD received her bachelor’s degree from The Ohio State University and her medical degree from Wright State University. She completed her family medicine residency program at University Hospitals and a preventative medicine fellowship with NFP. Dr. Golembiewski has a strong interest in maternal and neonatal care, global health and geriatrics.

Vice President - Behavioral Health and Care Integration
Peggy Keating, LISW-S
Peggy Keating joined NFP in 2011, assuming the position of vice president of behavioral health and care Integration. Peggy leads an incredible team of behavioral health clinical providers and support staff, with the goal to provide NFP patients with the behavioral health services that they need. Also, Peggy works to ensure NFP delivers integrated care, providing seamless and supportive care for all our patients, for all services. Peggy has long championed the integrated care model, which simply values care for the whole person and recognizes that behavioral healthcare is healthcare!
As a clinical social worker, Peggy has over 40 years of experience working with groups, families, and individuals in a variety of social service organizations throughout the greater Cleveland community. She has provided over 30 years of clinical and administrative leadership in roles with increasing responsibility. Peggy has presented at local and national conferences, served on professional panels and been a vocal advocate for those she serves.
Peggy earned her Bachelor of Arts degree, double majoring in social services and religious studies and a Master of Arts in psychology and a Master of Social Work, all from Cleveland State University. Peggy’s has always called Cleveland home, and loves living on the Northcoast!
Chief Compliance and People Officer
Candace LaRochelle
Candace joined NFP in November 2021 after working most recently as the area director of business development for University Hospitals Rehabilitation Hospitals.
At NFP, Candace’s efforts are focused on all aspects of patient care and outcomes using innovative solutions and strategies to promote organizational change, ensure compliance with state and federal regulations, improve patient care and more.
She earned her bachelor’s degree from Winston-Salem State University, a master’s of healthcare administration degree from Pfeifer University and a law degree from University of Dayton School of Law.
Candace and her husband Ludgy have two children and live in Highland Heights.
Chief of Staff and Vice President of Marketing and Communications
Andrea Lyons
Andrea joined Neighborhood Family Practice in August 2021 as vice president of marketing and communications. In this position, Andrea leads marketing and strategic communications with the goal to increase the visibility of the organization’s services and locations to current and prospective patients, as well as other community-based stakeholders and supporters.
Andrea has nearly 30 years of experience in marketing and communications across various for-profit and nonprofit organizations. Prior to joining NFP, Andrea was the director of race, diversity, equity and inclusion for United Way of Greater Cleveland. In that role, she was responsible for developing and executing diversity and inclusion programs for staff and across Cuyahoga and Geauga Counties.
Andrea earned her Bachelor of Arts in political science at Rutgers University and master of business administration from New York University – Stern School of Business.

Vice President of Clinical Transformation
Jason Schreiber
Jason Schreiber joined Neighborhood Family Practice in August 2024 as the Vice President of Clinical Transformation. In this role, Jason leads the organization’s value-based care and population health initiatives.
Jason earned his BA and MBA from Cleveland State University. He is a Fellow with the American College of Healthcare Executives and is Lean Six Sigma Black Belt certified. He most recently worked as the Director of Clinical Operations at Providence Community Health Centers in Providence, RI. Prior to that, he served as the VP of Operations & Population Health at Fisher-Titus and as an Operations Director at MetroHealth.
As a native Clevelander, Jason is excited to return home to develop systems and processes that advance health equity for the patients and communities we serve.
Chief Financial Officer
George J. Voss
George J. Voss is a seasoned finance professional with over 17 years of experience in financial management and accounting. He joined NFP as Chief Financial Officer in September 2024. Previously, he served as CFO at Crossroads Health, where he oversaw financial activities for a 300-employee organization, driving data-driven decision-making and strategic initiatives. George’s prior roles include CFO and Controller positions at One Health Ohio and Northeast Ohio Neighborhood Health Services, where he successfully managed multimillion-dollar budgets and federal grants.
Holding an MBA in Accounting from Baldwin-Wallace College and a Bachelor’s in Education from Kent State University, George is dedicated to fostering financial integrity and compliance. He has led transformative projects, including the transition to advanced accounting software and the optimization of 401(k) plans, resulting in enhanced operational efficiency and cost reductions.
In his free time, George enjoys spending time with his family and volunteering as a coach in local community sports programs.
