A message from President and Chief Executive Officer Domonic Hopson

On behalf of Neighborhood Family Practice, I am honored to present the 2023 annual report. As we reflect on 2023 achievements and challenges, I am filled with pride at the progress we have made and excitement for the opportunities ahead.

Last year, NFP remained resilient and adaptable, achieving significant success across all facets of our operations. This is thanks to the unwavering dedication of more than 200 colleagues with whom I have the honor and privilege to lead and support.

Our commitment to innovation, patient satisfaction and quality care has driven growth and improved health outcomes for our patients.

I am proud to share the following highlights:

Financial Performance: We are pleased to report that NFP is in a very positive financial position. Our continued focus on health outcomes through patient outreach and increased in-house pharmacy utilization has spurred revenue growth. This, coupled with our team’s innovative and calculated use of resources, has helped to control costs and increase profit margins.
Market Expansion: We are poised to increase access with the opening of the new W. 130th Community Health Center and a partnership with St. Vincent’s, in which we are expanding to Central neighborhood.
Innovation and Technology: Our investment in innovation and technology has provided access to cutting edge data that has improved our patient outcomes – particularly by race and ethnicity. Our treatment plans are tailored for each unique patient.

Our patients speak more than 50 different languages which can be a challenge in any given situation. As a solution, NFP introduced an online translation tool, “Mother Tongue Translator.” The tool, which is a first of its kind, is currently being used to assist individuals during birth who speak a language other than English.

Patient Focus: Patient satisfaction remains at the forefront of everything we do. We are here to serve all, regardless of their ability to pay. Every day we strive to be the health care solution of choice and not default by delivering exceptional value and service to everyone who enters our doors.

In 2023, we introduced a three-year strategic plan providing a clear path for our future.

This extensive research and effort resulted in a new vision, mission, and set of values that serve as guideposts for our work with each other, our patients, and our neighbors.

Inclusion
and Belonging

Our diversity, equity, inclusion, and belonging (DEIB) efforts are essential for our staff’s well-being and for achieving health equity for the patients we serve. The DEIB staff committee leads our internal initiatives, focusing on creating a welcoming environment where all individuals, regardless of their background or identity, feel valued, respected, and included. These efforts include cultural and identity-based celebrations and educational awareness for staff.

The board of directors continues to demonstrate its dedication to our organization’s commitment to diversity, equity, and inclusion. Their efforts include recruiting board members and selecting vendors who reflect our patient population.

Our Rich HISTORY

Since 1980, NFP has focused on providing high quality primary care and other services to residents of Cleveland’s near west side and surrounding communities, particularly those who lacked access to care.

From the early days with one office in a storefront where we grew to serve 8,000 patients to our present seven community health center locations in Cleveland and Lakewood that serve more than 21,000 patients, NFP’s experienced team of providers and staff remain dedicated to fulfilling.

The key to prevention of various illnesses and diseases lies in proactive outreach to patients who are most at risk. The NFP Population Health team had more than 20,000 patient touchpoints through phone calls, text messages, letters and patient record cleanup. These efforts resulted in the scheduling of thousands of appointments for screenings and immunizations to help prevent various diseases including breast cancer, diabetes, hypertension and colorectal cancer.

Engagement

Our Midwifery team hosts a Breast Milk Donation Drive for Members of the Community.

Our neighbors are incredibly important to us – it’s not just part of our name, it’s central to our mission.

We understand that health and well-being extend beyond the four walls of an exam room. That’s why we prioritize being out in the community, engaging with our neighbors and welcoming them into our centers to foster a strong, healthy community together.
