Neighborhood Family Practice (NFP) was founded in 1980.

In response to a concern that residents of Cleveland’s near west side lacked access to primary health care. A Federally Qualified Health Center (FQHC), NFP focuses on providing high quality primary care in a medical home setting.

As a community provider, our seven convenient locations offer same day appointments. The NFP service area spans twelve neighborhoods across Cleveland’s west side and Lakewood and is comprised of a very diverse mix of residents.

NFP accepts Medicaid, Medicare, most commercial insurance plans and offers a financial assistance program (sliding fee discount) for the uninsured.

NFP is a recipient of the Health Resources and Services Administration (HRSA) and each of its locations are covered by the Federal Tort Claims Act (FTCA). This health center receives HHS funding and has Federal Public Health Service (PHS) deemed status with respect to certain health or health-related claims, including malpractice claims, for itself and its covered individuals.

At Neighborhood Family Practice (NFP), our mission is to build healthy communities by providing trusted, high-value health care for all.

Since becoming a federally qualified health center in 2000, NFP has steadily expanded to meet the evolving needs of our community:

2005

Opened our second health center in Tremont.
Tremont Community Health Center

2012

Launched a third location in the Detroit Shoreway neighborhood.

2014–2015

Added two more locations—one in Puritas and another at West 117th and Lorain Avenue—driven by the Affordable Care Act and Medicaid expansion.

2017

Introduced dental services at the Ridge Community Health Center and expanded our Puritas site.

2019

NFP merged with North Coast Health in Lakewood, establishing our sixth location, North Coast Community Health Center. This partnership expanded services for Lakewood and surrounding communities and created the North Coast Health Foundation, which advances NFP’s mission through fundraising for charitable care, operations, and capital needs. That same year, we acquired the Dave’s Mercado Pharmacy on Ridge Road, now Neighborhood Family Practice Pharmacy, enhancing care continuity with easier access to prescription medications.

Later in 2019, we opened the Ann B. Reichsman Community Health Center, our seventh location. This center provides comprehensive care for pregnancy, children, and families, complementing services at the Ridge location.

2021

The North Coast Community Health Center moved to a larger facility in Lakewood’s historic Birdtown neighborhood, increasing access for residents in eastern Lakewood and Cleveland’s western edge.
