Michael Arnold, DO

RIDGE COMMUNITY HEALTH CENTER

Michael, a Canton, OH native, pursued his undergraduate studies in biology at Brown University. He then embarked on an intensive six-year accelerated training program in family medicine, completing it through Ohio University and Cleveland Clinic Akron General. Outside of his professional life, Michael is passionate about socks and cinema. He enjoys staying active, exploring new destinations, and cherishing moments with his puppy and fiancé.

Elizabeth Beach, CNM

Lizzie Beach began her career with the NFP team as a student midwife in 2018. She went on to work as a certified nurse midwife at Cleveland Clinic before rejoining the NFP midwifery team in 2023.

Originally from Chicago, Illinois, Lizzie holds a Master of Science in Nursing from Case Western Reserve University.

Passionate about supporting women throughout their healthcare journey, Lizzie’s areas of focus include low-intervention birth, postpartum care, and adolescent health. In her personal time, she enjoys outdoor activities with her husband and their three young children.

Brian Bouchard, MD

Detroit Shoreway Community Health Center

Brian Bouchard received his bachelor’s degree from Davidson College in 2012 and his medical degree from Washington University at the St. Louis School of Medicine in 2016. He completed his family medicine residency at The Christ Hospital in partnership with the University of Cincinnati. In 2019, Dr. Bouchard joined Neighborhood Family Practice in 2019 as a family physician.

Victoria Callahan, APRN, CNP

W.130th Community Health Center

Victoria brings 20 years of experience in healthcare to her role as a Family Nurse Practitioner. She earned her Master’s degree from Herzing University in 2024. As a Family Nurse Practitioner, Victoria is dedicated to providing care to patients of all ages. She is passionate about helping others and improving their well-being. Outside of her professional work, Victoria enjoys reading, crafting, exploring nature, and spending quality time with her husband and family.

Kathy Chilton, APRN-CNP

Tremont Community Health Center

Kathy Chilton joined Neighborhood Family Practice in 2019. A family nurse practitioner, she earned her bachelor of science in nursing and master of science in nursing from the University of Texas School of Nursing. Kathy has over 25 years of experience as a nurse and a decade of experience as a nurse practitioner. She has provided primary care services to adult and pediatric populations managing acute and chronic illnesses.

Austine Clark, CNP

RIDGE COMMUNITY HEALTH CENTER

Austine Clark joined Neighborhood Family Practice in 2023. She earned her BS in Biology from Loyola University Chicago. After serving in the Peace Corps, she began nursing school at Case Western Reserve University where she earned her MSN degree. Since then, she has worked in Cardiology, Pediatrics, and Family Health. Austine has a passion for travel, culture, and learning about how other people live. In her free time, she enjoys gardening, running, traveling and spending time with her husband and two children.

Stephanie Deuley, DO

RIDGE COMMUNITY HEALTH CENTER

Stephanie Deuley, DO received her medical degree from Ohio University Heritage College of Osteopathic Medicine in 2018. She completed residency training in 2021 at the Cleveland Clinic Family Medicine Residency in Lakewood, OH. Dr. Deuley started working at the NFP Ridge Community Health Center in August 2021. She enjoys seeing patients of all ages with other interests in women’s health, adolescent care, LGBTQ+ health, global health, and performs osteopathic manipulative therapy (OMT). Otherwise, Dr. Deuley enjoys traveling, running, and reading in her spare time.

James Diekroger, MD

Puritas Community Health Center

James Diekroger, MD graduated from the Ohio State University College of Medicine in Columbus, Ohio. He completed his residency training at the University of Rochester Highland Hospital in Rochester, New York. His areas of interest include prevention and education, as well as integrative medicine. He speaks French and a little Spanish. He is married and enjoys disc golf, as well as movies and books, especially those that involve science fiction.

Mary DiGeronimo, APRN-CNP

NORTH COAST COMMUNITY HEALTH CENTER

Mary DiGeronimo joined Neighborhood Family Practice in 2022 as a family nurse practitioner. She received her bachelor’s degree of nursing from Ohio State University and her master’s degree from Simmons University. Her nursing background includes pediatrics and virtual health. She is dedicated to ensuring health care for all with particular interest in care for children, adolescents and member of the LGBTQ2+ community. She also has a strong interest in environmental health and integrative medicine.

Katherine Donofrio, DNP, APRN, FNP-BC

Katherine Donofrio joined Neighborhood Family Practice in 2023 as a family nurse practitioner. She earned her bachelor of arts in psychology at University of Dayton, a bachelor of science in nursing at Baldwin Wallace University and her doctorate of nursing at Cleveland State University. Her nursing background includes digestive disease transplant and infant/maternity. She is passionate about providing safe and compassionate care to all patients with a strong interest in supporting growing families and advocating for mental health. Katherine enjoys local adventures and travel, while spending time with her family.

Amy Ebbitt, APRN-CNP

Amy Ebbitt, family nurse practioner, received her bachelor’s degree from Wheaton College in Anthropology and a master of science in nursing from Case Western Reserve University. Amy has worked at Neighborhood Family Practice since 2009. Prior to working at NFP, Amy worked in Infectious Disease and Women’s Health at University Hospitals. Amy is committed to ensuring healthcare access for all. Amy enjoys spending time with her family in her free time.

Rene Fitzpatrick, APRN-CNP

Puritas Community Health Center

Rene Fitzpatrick is a family nurse practitioner who specializes in the care of individuals and families. Prior to joining NFP in 2022, she worked at various area health care facilities providing specialized wound care and health management. Rene earned her bachelor of science degree in nursing from Ohio University and her masters from Walsh University. In her spare time, Rene enjoys cooking – often trying new recipes to share with others – and spending time with her family.

Prakash Ganesh, MD, MPH

Ridge Community Health Center

Prakash Ganesh earned his medical degree from the Medical School for International Health at Ben Gurion University in collaboration with Columbia University located in Beer Sheva, Israel and his master’s in public health from Case Western Reserve University School of Medicine. He has worked on and off at NFP since his residency in 2013 and became a staff physician in 2020. Dr. Ganesh has an interest in HIV care, refugee care and global health and is certified as an American Academy of HIV Medicine Specialist. In his free time he enjoys the spending time with his wife and two daughters hiking and biking.

Melanie Golembiewski, MD, MPH

W. 130th Community Health Center

Melanie Golembiewski, MD, joined Neighborhood Family Practice (NFP) in 2010. She earned her bachelor’s degree from The Ohio State University and her medical degree from Wright State University. Dr. Golembiewski completed her family medicine residency at University Hospitals and a preventive medicine fellowship with NFP.

Her clinical interests include maternal and neonatal care, global health, and geriatrics. Dr. Golembiewski also serves on the Cuyahoga County Women’s Health Commission.

Kathy joined NFP in 2024 as a Women’s Health Nurse Practitioner. She earned both her Bachelor of Science in Nursing and her Master of Science in Nursing from Kent State University. A lifelong resident of Cleveland, Kathy is deeply committed to providing compassionate care to women and advocating for the reduction of healthcare disparities. Outside of her professional work, she enjoys spending time with family and friends, reading, hiking, walking, and practicing yoga.

Mandy Healey, MSN, APRN, FNP-BC

Mandy Healey joined Neighborhood Family Practice in 2019. With a passion for public health and disease prevention, Mandy began her career as an Army Medic and served for almost 15 years with the Ohio Army National Guard. Mandy’s first degree was in Zoology from the Ohio State University, and after moving to Cleveland she earned her MSN as a Family Nurse Practitioner at Case Western Reserve University. In her spare time, Mandy loves running and spending time with her husband, 4 children and animals.

Erick Kauffman, MD, MPH

Dr. Kauffman earned his medical degree from the University of Pittsburgh in 1997. He did his training in family practice at the University of Washington in Seattle, Washington. He came to Neighborhood Family Practice in 2000 to fulfill his service commitment with the National Health Service Corps and has made NFP his career. He enjoys seeing patients of all ages, including prenatal care. He also speaks Spanish. Dr. Kauffman is married, has three children and enjoys family and outdoor activities.

Julie A. DuBose (Kellon), APRN, CNM

Julie Kellon, midwife, joined Neighborhood Family Practice in 2010. Julie obtained her BSN in Nursing from Case Western Reserve University in 1995. She received her MSN in Nursing from Case in 2000 and completed her Midwifery training at the Frontier School of Midwifery and Family Nursing. Julie is licensed as a Certified Nurse Midwife by the State of Ohio and a Member of the American College of Nurse Midwives. Julie’s areas of interest include obstetrics and gynecology, colposcopy and sexual health.

Joseph Labastille, MD

Joseph Labastille, MD received his medical degree from the State University of Haiti. He has over 30 years of experience in family medicine. Dr. Labastille also sees patients at Ridge Community Health Center.

Lauren Lasko, APRN-CNP, IBCLC

Lauren received her training from Columbia University’s School of Nursing in NYC and has worked at federally qualified health centers since she began her career. Lauren is passionate about women’s health and breastfeeding medicine, she is also proficient in Spanish. She continues to grow a breastfeeding medicine program at NFP.

Kate Lawrence, APRN, CNM, IBCLC

Kate Lawrence, midwife, joined NFP in 2015 as a certified nurse midwife. Before joining our team, Kate worked as a clinical nurse at the Cleveland Clinic’s Family Birthing Center and a labor and delivery nurse at MetroHealth. Kate received a Bachelor of Science in Nursing from the University of Akron and a Master of Science in Nursing/Nurse-Midwifery from Frontier Nursing University. Kate completed her Nurse Midwife- Clinical Practicum at Paragon Health Associates. She also serves as a visiting professor-maternal child nursing at Chamberlain College of Nursing and an adjunct clinical instructor- maternal child nursing at Cuyahoga Community College.

Nancy Li, MD

Puritas Community Health Center

Nancy Li joined Neighborhood Family Practice in 2019 as a family physician. She earned her bachelor’s degree from Vanderbilt University in 2012 and her medical degree from University of Tennessee Health Science Center in 2016. Dr. Li completed her family medicine residency at University Hospitals Cleveland Medical Center in 2018.

Katy Maistros, APRN, CNM

Katy Maistros, midwife, is honored to be working with the family practice providers and fellow nurse midwives at Neighborhood Family Practice. She has been at NFP since 2005 and feels privileged to serve the women of the Near West Side as a Nurse Midwife. Katy grew up in Hicksville. Ohio and graduated from Bowling Green State University with her Bachelor’s Degree in Nursing, She was a Peace Corps Volunteer from 1999 to 2001 in Turkmenistan, Central Asia. Katy graduated from Case Western Reserve University with a Master’s in Nurse-Midwifery.

She currently lives on the west side of Cleveland with her husband and three sons. Her philosophy of care encourages women to recognize their highest potential for physical, spiritual, and emotional health. Katy’s interests lie in low interventive childbirth, adolescent health, and family planning.

Lisa Navracruz, MD

Detroit Shoreway Community Health Center

Dr. Lisa Navracruz joined Neighborhood Family Practice in 2019. She received a bachelor of science degree from Boston College and a medical degree from Stanford University’s School of Medicine. In addition to her experience as a family physician for over 13 years, Dr. Navracruz’s clinical practice has been focused on HIV Primary Care in a FQHC setting. She is certified as an American Academy of HIV Medicine Specialist. Dr. Navracruz is also an assistant professor of family medicine at the Center for Medical Education at Case Western Reserve University.

Jordan Nelson MSN, APRN-CNP, FNP-C

Jordan Nelson joined Neighborhood Family Practice in 2024 as a Family Nurse Practitioner. He earned his Bachelor of Science in Nursing from Cleveland State University and Master of Science in Nursing from Case Western Reserve University. He also previously served in the US Army Reserve as a preventive medicine specialist. He sees patients of all ages and emphasizes the prevention of chronic disease and its progression. In his free time, he loves being outdoors, gardening, woodworking, and spending time with his family.

Natalia Parkanzky, APRN-CNP

Natalia Parkanzky, family nurse practioner, received her Bachelor of Science in Clinical Laboratory Science from Miami University and a Master of Science in Nursing from Case Western Reserve University. Natalia grew up and currently resides on the west side of Cleveland. She enjoys encouraging patients of all ages to maintain a healthy lifestyle with daily exercise and a balanced diet. She speaks Spanish and Ukrainian. Her outside hobbies include running, baking desserts, and dancing and singing with her children.

Aleksandra Rabinovich, APRN-CNP

Aleksandra Rabinovich, family nurse practioner, holds a master of science in nursing, and received her education and training as a family nurse practitioner from The Ohio State University College of Nursing. Aleksandra has professional experience as a family nurse practitioner in both outpatient and inpatient settings, and has previous experience as an emergency nurse in an urban trauma center. She has a passion for preventive care. Aleksandra speaks fluent Russian, and in her free time, she enjoys salsa dancing and spending time with her family.

Alexis Rayokovich, PA-C

Alexis Rayokovich joined Neighborhood Family Practice in 2025 as a Physician Assistant (PA-C). She received her bachelor’s degree from Ohio State University and her Masters of Medical Science in Physician Assistant studies from Midwestern University. During her education, she developed a strong passion for compassionate patient care and education. Her clinical interests include social determinants of health, patient education, pediatrics, and LGBTQ+ care. Outside of work, Alexis enjoys staying active and spending time with family and friends.

Nadia Robinson, APRN, CNM, FNP

Nadia joined Neighborhood Family Practice as a certified nurse midwife in 2020. She first connected with NFP as a volunteer birth and postpartum doula (Nadia is a cofounder of the Northeast Ohio Doula Collective). She received her bachelor’s degree from Earlham College in 2010, and graduated from the Vanderbilt University School of Nursing’s Nurse Midwife and Family Nurse Practitioner program in 2020. Nadia’s favorite part of being a midwife is witnessing people learn about themselves, discover their ability to heal and experience their power. In her free time she loves to read, sing, dance, occasionally paint, and spend time with the people she loves.

Sandra Sauereisen, MD, MPH

Sandra Sauereisen, MD MPH received her bachelor’s degree from Northwestern University, her medical degree from Hahnemann University and an MPH from Johns Hopkins University Bloomberg School of Public Health. She completed her Family Medicine residency in Philadelphia at the Thomas Jefferson University Hospital. Since 1998, she was the Director of Community Medicine and full-time faculty with the UPMC St. Margaret Family Medicine Residency until she moved to Cleveland in 2021 when she joined NFP. Sandy’s passion is caring for marginalized populations to ensure access to dignified, respectful, empathic primary care that promotes self-efficacy and focuses on wellness, not just disease management. She is married, has 3 children and enjoys the outdoors and spending time with family.

Ruth Schulz, CNM

Ruth joined Neighborhood Family Practice as a certified nurse midwife in 2024. She earned her Master of Science in Nursing from Case Western Reserve University and has nursing experience at the Cleveland Clinic’s Women’s Health Institute and Fairview Hospital. Passionate about empowering women through informed care, she has special interests in holistic health, physiological birth, and menopause management. Ruth lives on Cleveland’s west side with her family and enjoys traveling, running in the Metroparks, and rowing with her crew mates.

Jennifer Sobrowski, APRN-CNP

Jennifer Sobrowski joined NFP in 2017 as a family nurse practitioner. Jennifer earned her master of applied sciences in nursing from the University of Memphis and her bachelor of science in nursing from Cleveland State University. She has experience various clinical, family medicine and behavioral health settings. Jennifer dedicates some of her free time to rescuing and fostering animals to find them loving homes. Jennifer also sees patients at Ridge Community Health Center.

Anne Wise, MD, MPH

Detroit Shoreway Community Health Center

Anne Wise, MD received her medical degree from Temple University School of Medicine. She completed her residency in family medicine at St. Francis Hospital in Hartford, Connecticut, followed by a fellowship in Occupational and Environmental Medicine at the University of Connecticut Health Center in Farmington. Dr. Wise has practiced at Neighborhood Family Practice since 1995. Fluent in Spanish, her other interests include prison medicine, immigration policy, advocating for national health insurance and mystery novels (lots of them!)

Sasha Yurgionas, MD
